If you are curious about your insurance plan’s nutrition benefits, feel free to call your insurance provider and ask them these questions:

  1. Does my plan cover outpatient nutrition counseling (procedure codes 97802 & 97803)?

  2. If yes, how many sessions are allowed with the diagnosis code z71.3 (general nutrition counseling)?

  3. Do I have a deductible to meet before insurance pays?

  4. Will I have a co-pay or co-insurance?

  5. Do I need a physician referral?

  6. Is this provider in network: Martha Flores, NPI: 1780301044

  7. If no, what are my out of network benefits?
